Consent to the proccessing of personal data in the future recruitment processes / Zgoda na przetwarzanie danych w przyszłych rekrutacjach

I hereby consent to processing of my personal data provided in the documents and forms sent by me and collected during the recruitment procedure by Addepto sp. z o.o. with its registered office in Warsaw, at Świeradowska Str. 47, postal code 02-662, KRS no. 0000729066 (hereinafter referred to as "Controller"), for the purposes of future recruitment processes carried out by the Controller from the date of giving my consent for a period of the next 36 months counted from the end of the calendar year in which I gave my consent or until my consent is withdrawn.

I have received information about the data processing by Controller, including the following:

- my consent is voluntary;

- I can withdraw my consent at any time, but I am aware, that withdrawal of consent does not affect the lawfulness of the processing prior to the withdrawal;

- I have the rights, in particular, to access data, to receive a copy of the data, to rectify them, to delete, to limit processing, to raise complaints to the supervisory authority, and to data portability.

Zgadzam się na przetwarzanie moich danych osobowych zawartych w CV, powyższym formularzu oraz zebranych w toku postępowania rekrutacyjnego przez Addepto sp. z o.o. z siedzibą w Warszawie do celów przyszłych procesów rekrutacyjnych przez okres kolejnych 36 miesięcy liczonych od końca roku, w którym wyraziłem/am zgodę lub do jej odwołania.

Otrzymałem/am informację o tym, jak będą przetwarzane moje dane, w tym, że:

- moja zgoda ma charakter dobrowolny;

- przysługuje mi prawo do jej wycofania w każdym czasie, co nie będzie miało wpływu na zgodność z prawem przetwarzania danych przed wycofaniem zgody;

- przysługują mi prawa, w tym prawo dostępu do danych, otrzymania ich kopii, sprostowania, przenoszenia, usunięcia, żądania ograniczenia ich przetwarzania, wniesienia skargi do organu nadzorczego.
